Making a draft of Questionairre

For the research purpose, researchers have to come up with a draft.

The questionnaire must consist of the logo of organisation/ institute and etc, followed by the title of the research. Next, a small introduction is given to explain the respondent about the research and give the objective of the research. Follow by personal information of the researcher, example (ID number, degree). Furthermore insert the instruction for the respondent on how to answer the question. Example is either to circle or tick only one answer. Next, the questionnaire must contain the demography part of respondent where age, gender, career, etc will be asked but not the name. After that, insert the questions for the research followed by appreciation for cooperation.
